Existing Patients

Use the Billing Portal to make a payment, send appointment requests, submit demographic updates or access patient registration forms. If you have any questions regarding payment, please call us at 208.376.4265 or email us at billing@idahoderm.com so we can quickly help you. 

Use the Patient Portal to access lab results and medical records, request medication refills, or update health history.

New Patients

To save yourself time in the office we encourage you to complete these new patient forms before you come in for your first appointment. 

If you would prefer to do all of your paperwork electronically, call the office and ask our scheduling desk to activate your account on our secure portal. They will need to confirm your email address to create an account. This must be done at least 24 hours in advance of your appointment so our staff can process your information. 

Please print and complete the forms below if you are unable to access our secure patient portal. You will need the below Patient Forms that include a Patient Information form, Medical History form, Credit and Cancellation Policy, and HIPAA Privacy Policy, which all need to be completed before your first visit. You can print, sign, and fax the Patient Forms to 208.376.4265 or email them to mail@idahoderm.com.

If your insurance covers dermatological treatments, you will receive the benefit of reduced out-of-pocket costs. Because insurance policies vary, we review each case individually to determine the best course of action for you. Additionally, some plans require a referral from your primary care physician so it’s a good idea to check with your insurance company for specific requirements.

Please check with your individual insurance carrier for coverage. 

The Dermatology Clinic of Idaho will bill most carriers on behalf of our patients. However, our office can never guarantee your personal coverage or benefits. Only your insurance company’s customer service department can guarantee that our providers are in their network.
